-Summary

This study will compare the use of 27-inch and 55-inch endoscopic imaging monitors during minimally invasive surgery for women with endometrial cancer. Participants will be randomly assigned to one of the two monitor groups. All participants will undergo the same standard surgical procedure; only the monitor size used during surgery will differ. The main question is whether monitor size affects total operative time. The study will also evaluate sentinel lymph node dissection time, blood count changes, complications, infections, hospital stay, and drain-related outcomes during the first 30 days after surgery.

+Detailed description

This is a single-center, prospective, randomized, parallel-group clinical study designed to evaluate whether endoscopic imaging monitor size affects surgical and early postoperative outcomes in minimally invasive endometrial cancer surgery. Eligible adult women with histologically diagnosed endometrial cancer who are scheduled for total laparoscopic hysterectomy with bilateral salpingectomy or salpingo-oophorectomy and bilateral sentinel lymph node dissection will be enrolled after informed consent.

Participants will be randomly assigned in a 1:1 ratio to surgery using either a 27-inch endoscopic imaging monitor or a 55-inch endoscopic imaging monitor. Both monitor systems are standard surgical imaging equipment used in routine clinical practice. The surgical procedure, operative technique, energy devices, sentinel lymph node mapping approach, and postoperative care will be standardized across both groups. No additional invasive procedure, investigational drug, biologic product, or experimental treatment will be used as part of this study.

The primary outcome measure is total operative time, defined as the time from entry of the laparoscopic optic into the abdomen after trocar placement to completion of surgery. Secondary outcomes include bilateral pelvic sentinel lymph node dissection time, number of right and left sentinel lymph nodes removed, hemoglobin and hematocrit decrease, blood transfusion requirement, intraoperative complications, postoperative complications within 30 days, postoperative infections within 30 days, Clavien-Dindo complication grade, length of hospital stay, drain requirement, and postoperative day of drain removal if a drain is placed.

Participants will be followed during hospitalization and at postoperative day 7-10 and day 30 visits. The study aims to determine whether monitor size has a measurable effect on operative performance and early postoperative outcomes in a standardized minimally invasive surgical setting.

27-Inch Monitor GroupExperimentalDevice: 27-Inch Endoscopic Imaging MonitorParticipants randomized to this arm will undergo minimally invasive endometrial cancer surgery using a 27-inch 4K-2D endoscopic imaging monitor. The surgical procedure and perioperative care will be standardized.
55-Inch Monitor GroupActive comparatorDevice: 55-Inch Endoscopic Imaging MonitorParticipants randomized to this arm will undergo the same minimally invasive endometrial cancer surgery using a 55-inch 4K-2D endoscopic imaging monitor. +Outcome measures

Primary outcomes

  • Total Operative Time [During surgery]: Total operative time, defined as the time from entry of the laparoscopic optic into the abdomen after trocar placement to completion of surgery, measured in minutes.

Secondary outcomes

  • Bilateral Pelvic Sentinel Lymph Node Dissection Time [During surgery]: Time required to complete bilateral pelvic sentinel lymph node dissection, measured in minutes.
  • Number of Retrieved Sentinel Lymph Nodes [From surgery to final pathology assessment, up to 30 days after surgery]: Number of right and left sentinel lymph nodes removed during surgery.
  • Hemoglobin Decrease [Preoperative to postoperative 24 hours]: Percentage decrease in hemoglobin level calculated using preoperative and postoperative 24-hour hemoglobin values.
  • Hematocrit Decrease [Preoperative to postoperative 24 hours]: Percentage decrease in hematocrit level calculated using preoperative and postoperative 24-hour hematocrit values.
  • Blood Transfusion Requirement [From start of surgery to hospital discharge, up to 30 days after surgery]: Need for perioperative or postoperative blood transfusion, recorded as yes or no.
  • Intraoperative Complications [During surgery]: Occurrence of any intraoperative complication, recorded as yes or no.
  • Postoperative Complications Within 30 Days [Up to 30 days after surgery]: Occurrence of any postoperative complication within 30 days after surgery, recorded as yes or no.
  • Postoperative Infections Within 30 Days [Up to 30 days after surgery]: Occurrence of postoperative infection within 30 days after surgery, recorded as yes or no.
  • Clavien-Dindo Complication Grade [Up to 30 days after surgery]: Severity of postoperative complications classified according to the Clavien-Dindo classification
  • Length of Hospital Stay [From surgery to hospital discharge, up to 30 days after surgery]: Duration of postoperative hospital stay, measured in days.
  • Drain Requirement [From surgery to hospital discharge, up to 30 days after surgery]: Need for postoperative drain placement, recorded as yes or no.
  • Postoperative Day of Drain Removal [From surgery to drain removal, up to 30 days after surgery]: Postoperative day on which the drain is removed in participants with a drain placed
